805 Physical Therapy and Performance is a small, one-on-one, cash-pay physical therapy practice in Ventura, CA. We keep this simple: we only collect what we need to get back to you and set up a visit, and we never sell or share it.
What we collect
If you fill out the “Request a visit” form, we collect the details you enter:
- Your name
- Your phone number
- Your email address
- Your visit preferences (in-clinic or in-home/mobile, and the days and times that work for you)
- Any notes you add about what's going on
If you call, text, or email instead, we'll simply have whatever you choose to share that way.
How we use it
We use your information for one thing: to respond to you and schedule your visit. That's it. We do not sell, rent, or trade your information, and we don't share it with anyone for marketing or send marketing emails.
Cookies & analytics
This website may use analytics tools (such as Google Analytics) to understand basic, anonymous traffic — like how many people visit and which pages they look at. This helps us improve the site. Any such tool may set cookies; it isn't used to identify you personally, and you can block or clear cookies in your browser at any time.
How your information is stored
Requests sent through the form are processed by Web3Forms (a third-party form service) and delivered to Sandy by email; the website itself is hosted on Cloudflare Pages. We keep them only as long as we need to schedule and follow up with you.
A note on medical records & HIPAA
This website and the request form are just a way to get in touch — they are not a place to send medical records or protected health information, so please don't include sensitive medical details in the form. This site is not a substitute for the HIPAA-protected handling of medical records; any health information that's part of your actual care is handled separately and appropriately.
805 PT is cash-pay and out-of-network — we don't bill insurance, so no insurance, claims, or payer information is collected or processed through this website.
